12/14/2023 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home, Ms. Broadnax appeared to be in a foul mood evident by her facial expressions such as a frown on the face. QMHPA inquired about her morning. She verbalized to QMHPA that her morning was going terrible but she would get through the day. QMHPA discussed with Ms. Broadnax that this session was going to focus on feelings of depression and being able to utilize learned coping skills to assist with those feelings. Ms. Broadnax did not present any symptoms of SI or HI. QMHPA began the session by inquiring about Ms. Broadnax current thoughts, feelings and emotions. QMHPA educated Ms. Broadnax on productive ways to reduce and begin to eliminate stress through developing healthy relationship with others. QMHPA encouraged Ms. Broadnax to verbalize her thoughts, feelings and emotions rather than isolating herself from others. QMHPA reinforced to Ms. Broadnax the importance of learning to communicate respectfully and interact without becoming verbally abusive and socialize with other in a positive manner without becoming aggressive. QMHPA taught Ms. Broadnax some skills she could explore to help reduce symptoms associated with depression such as exercise, deep breathing and relaxing. Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session today. Ms. Broadnax shared that she has been experiencing stress, depression and anxiety since she woke up. Ms. Broadnax was able to identify triggers, length of time she was depressed and the severity of her depression, anger and anxiety. Ms. Broadnax informed QMHPA that she did not take her medication last night because she just didnât want to. QMHPA reinforced the importance of a daily medication regime. Ms. Broadnax was also receptive to sticking to a medication regime that will support her mental health. Ms. Broadnax stated to QMHPA that she just needs that extra push sometimes from someone who knows what one is going through and a little professional help is always good. She stated to QMHPA that she would continue to use the skills that QMHPA is teaching her when she is frustrated.
